Here's a card I made after seeing one like it by Jen del Muro. She didn't have any instructions or template, so I made up one to share with you.
The finished card measures 4 1/4" x 5 1/2".

Here's a little tutorial on how to put it together.
You'll need:
2 strips each measuring 1/2" x 5 1/2"
3 pickets each measuring 1 9/16" x 4 1/4"
1 card base measuring 3 1/8" x 5 1/2
Here are the pieces already sponge daubed with CTMH Bamboo Ink.

Now you'll glue the two longs strips to your pickets. It's important to remember to leave a little extra space on the left side so that you can fold open your card.

I left ~1/8" space in between my pickets.
